Major Achievements 2014
(1) Identification of three focus area:
· The role of early patient education within a best practice model of care
· Improved education and awareness about home dialysis for all healthcare professionals
· The role of new technologies and initiatives to better support home dialysis patients
(2) Completion of the HOME Network Strategic Plan including elements of to drive our ability to successfully achieve our vision.
(3) Representation at the Home Therapy Conference in Melbourne with opportunities to showcase our vision and achievements.
(4) Recruitment of four (4) new members
(5) Completion of a number of online surveys to capture national data
(6) Published a number of information and education material to support the renal community.
(7) Development of HOME Network Website and Go Live on 1 November 2014
(8) Commencement of the multi-centre research study titled: Identifying Patient Related Factors Associated with Uptake of Home Dialysis and Patient Outcomes.
