Current projects
The HOME Network is currently working on projects aligned with its three focus areas from 2014 onwards.
1) Improved awareness, knowledge and education about home dialysis for healthcare professionals.
1) Improved awareness, knowledge and education about home dialysis for healthcare professionals.
- Since 2014, the group has identified, developed and implemented an educational package and approach that aims to address gaps in multidisciplinary healthcare professional understanding about home dialysis, empowering them with knowledge such that they are better positioned to help educate Australians with chronic kidney disease about home dialysis.
- Since 2014, the group has developed a flexible and culturally sensitive patient-centred ESKD pathway including education to inform any national group or organisation of the preferred key outcomes and patient journey that their RRT programme should aim to deliver, including optimal positioning of home dialysis within the pathway. - Since 2014, the group has explored, developed and implemented clinical and educational initiatives that exploit latest advances in technology (including communication, education and dialysis technologies) to strengthen partnerships with Australians with chronic kidney disease who dialyse at home.
As well as projects within the focus area groups, we also have a couple of other projects that sub-committees or individuals within THN continue to work on:
- The multi-centre research project "Identifying patient-related factors associated with uptake of home dialysis and patient outcomes" which was completed in 2017.
- Ongoing update and maintenance of the financial fact sheets for each state/territory.
